Here is a duplicable student application form document to help you organize your student applications.
The form covers personal information, reasons for applying, and trip expectations. There are spaces on the form for you to fill in specific dates, locations, and so forth, so read it carefully before you distribute it. You can add or change any question you like, but make sure that you always ask students to support their answers with examples from their lives – past behaviour predicts future behaviour! There are also spaces on the form for students to upload passport photos, COVID-19 QR codes, insurance details, and vaccination data. For this reason, you’ll have to make sure your students sign in using their Microsoft school accounts; otherwise, they won’t be able to upload the files. Once the applications are in, you can export the students’ answers to an Excel sheet, which you can print or keep as a digital copy to take with you on your trip.
